Provides youth and family training and support services to individuals with mental illness ages 18-21 and individuals with serious emotional disturbance ages 4-17 years who meet a hospital level of care. This waiver operates with a concurrent 1915(b)(1), 1915(b)(3), and 1915(b)(4) authority.
Provides adult day services, case management, homemaker, home health aide, personal support services, respite, skilled nursing, assisted living facility services, assistive technology, companion services, environmental modifications, home delivered meals, non-medical transportation, personal emergency response system, transition intensive case management, and transition setup services to individuals ages 65 or older and individuals with other disabilities ages 19-64 years who meet a nursing facility level of care.
Provides adult day services, case management, community living services, community support services, homemaker, personal care, respite, supported employment, dietician services, occupational therapy, physical therapy, skilled nursing, speech/hearing/language services, behavioral support services, child habilitation services, cognitive retraining, companion services, crisis intervention support, environmental modification, individual habilitation training, special family habilitation home, specialized equipment, and transportation services to individuals with brain injury ages 21 or older, and individuals with intellectual disabilities or developmental disabilities ages 0 or older who meet a nursing facility or ICF/IID level of care.
