Provides adult day health, case management, specialized respite, financial management services, participant directed coordination, attendant care, environmental and minor home adaptation, goods and services, home and community supports, home delivered meals, and non-specialized respite services to individuals with physical or other disabilities ages 0-64 years and individuals ages 65 or older who meet a nursing facility level of care.

Provides adult day training, case management, personal care, respite, supported employment, behavioral services, counseling, group counseling, occupational therapy, specialized medical equipment, speech therapy, community guide, companion, environmental and minor home modifications, financial management, goods and services, supervised residential care level I, supervised residential care level II, and supervised residential care level III services to individuals with brain injury ages 18 or older who meet a nursing facility level of care.

Provides adult day health, adult day training, case management, respite, supported employment, counseling, group counseling, nursing supports, occupational therapy, physical therapy, specialized medical equipment, speech therapy, behavioral services, community guide, community living supports, environmental and minor home modifications, family training, financial management, goods and services, supervised residential care level I, supervised residential care level II, and supervised residential care level III services to individuals with brain injury ages 18 or older who meet a nursing facility level of care.

Provides adult day health, adult day training, case management, respite, supported employment, counseling, group counseling, nursing supports, occupational therapy, physical therapy, specialized medical equipment, speech therapy, behavioral services, community guide, community living supports, environmental and minor home modifications, family training, financial management, goods and services, supervised residential care level I, supervised residential care level II, and supervised residential care level III services to individuals with brain injury ages 18 or older who meet a nursing facility level of care.

Provides skilled services by a licensed practical nurse, skilled services by a registered nurse, and skilled services by a respiratory therapist to individuals who are technology dependent ages 0 or older who meet a nursing facility level of care.

Provides case management, consultative clinical and therapeutic - nutritional services, consultative clinical and therapeutic - psychological services, consultative clinical and therapeutic services - positive behavior supports, day training, personal assistance, residential support level I, respite, shared living, supported employment, financial management services, community access, community guide, community transition, environmental accessibility adaptation services, goods and services, natural supports training, positive behavior supports, residential support level II, specialized medical equipment and supplies, technology assisted residential, transportation, and vehicle adaptation services to individuals with intellectual disabilities or developmental disabilities ages 3 or older who meet an ICF/IID level of care.
